The five steps
Per the FDA Wegovy prescribing information, weeks 1–4: 0.25 mg weekly. This is a starter dose — most patients see appetite changes but little weight loss yet. Weeks 5–8: 0.5 mg. Weeks 9–12: 1.0 mg. Weeks 13–16: 1.7 mg. Week 17+: 2.4 mg as the maintenance dose for weight management (Wegovy) or up to 2.0 mg for diabetes (Ozempic), per the Ozempic label.
When slower is better
If side effects are pronounced — particularly nausea that lasts more than 4 days per week — holding each step for 6 or 8 weeks instead of 4 is well tolerated and produces equivalent long-term weight loss. Pivotal trial data in STEP-1 (NEJM) showed no clinical urgency to reach the maximum dose to achieve meaningful response.
Missed doses
The 5-day rule: if you remember within five days, take the dose and stay on your schedule. After five days, skip it and resume on the next scheduled day. Never double up; two doses within five days can produce severe GI symptoms.
