  1. Understand the mechanism

GLP-1 is an incretin hormone your gut releases after meals. The therapy mimics that signal — slowing gastric emptying, increasing satiety, and modulating insulin. Knowing the mechanism makes every other decision (dose, timing, food) much easier.

  2. Pick the right medication

Semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro/Zepbound) are the dominant options. They differ in mechanism, efficacy, and label — and the right choice often depends on insurance, comorbidities, and tolerance.

  3. Master the titration ramp

Almost all side effects are dose-related. The standard semaglutide ramp moves through five doses over 16+ weeks. Going slower than the label is normal and often improves outcomes.

  4. Build the nutrition foundation

Appetite drops fast. The risk in week 1–8 isn't eating too much — it's not eating enough protein. Aim for 1.2–1.6 g/kg/day, spread across 3–4 meals.

  5. Anticipate the most common side effects

Nausea is universal and time-limited. Knowing what's normal, what isn't, and when to call your prescriber is the single biggest factor in staying on therapy.
